Please click on one of the forms below...Use NEW CLIENT if you have not been to Bridgeview before...and use CURRENT CLIENT if you have already been to Bridgeview Animal Hospital and set up an account with us. When you submit the form, it is immediately e-mailed to our office, and one of the staff will call you back promptly to schedule a time for your visit with us. Our office is open 7am to 6pm Monday through Friday and 8am to Noon on Saturday. We are usually able to get you an appointment with Dr. Graves either the same day or the next day if your pet needs to be seen that quickly. We look forward to seeing you for your appointment!
